Your Initial Visit
Paperwork
On your first visit, you will need to complete a registration form, and a medical history form. To help make the most out of your consultation time, please feel free to download and complete these forms ahead of time.
When you check-in, we will also ask to make photocopies of your:
- Driver's License (or other major ID)
- Medical insurance card (if available)

Parking
There is underground parking and validation is provided for the duration of your office visits and procedures.
Consultation fee
Consultations are billed to your insurance if applicable. If no medical insurance is available, there will be a $250 consultation fee due on the day of the visit.
Cancellation policy
We respect the value of your time and ask the same of you. Therefore, we request the courtesy of a 48 hour advanced notice for rescheduling or canceling your appointment. If this request is not honored, we reserve the right to ask for pre-payment of your fee by credit card prior to rescheduling another appointment for you. Consultations with Dr. Naficy require pre-payment of the $75 consultation fee, and this payment is fully refundable with a 48 hour notice of rescheduling or cancellation.
Privacy
The privacy of your personal and medical information will be fully maintained by our office. Also, during the course of your surgical procedure and subsequent visits, we will arrange for your entry and exit through a private door to the office.
